DUI EVALUATIONS/ASSESSMENTS
For individuals seeking a DUI Evaluation and/or Drug & Alcohol Assessment, a person may initially be evaluated in the Minimal Risk, Moderate Risk, Significant Risk, or High Risk or Early Intervention ASAM 1.5 category, which is based on driving history, prior offenses, drinking or substance use patterns, and other clinical or psychological factors.

Our staff is highly trained in Illinois DUI evaluation standards, court requirements, and ASAM criteria. Based on the results of the evaluation/assessment and any court or Secretary of State requirements, RockWater Counseling Center will recommend and place individuals into the appropriate level of care.

At this time, RockWater Counseling Center is offering court mandated group services for Minimal Risk, Moderate Risk, Significant, High Risk, ASAM .05, ASAM 1 and ASAM 2.1 (Intensive Out-Patient) levels of care.
Required Documents & Evaluation Process 
Before your DUI Evaluation/Assessment can be completed, certain documents are required. These documents help ensure the evaluation is accurate and completed in accordance with Illinois Secretary of State and court requirements.

Documents may include:

  • Current Illinois Driving Abstract/driving record (We would need a driving record from every state in which you had an active driver's license)

  • Police report or sworn report

  • Breathalyzer/BAC results or toxicology reports

  • Court documents related to the arrest(s) (if charge is non-DUI related)

If you do not currently have these documents, our staff can assist you in determining what is needed and where to obtain them. In many cases, clients can request their driving abstract online or directly through the Illinois Secretary of State. Obtaining the proper documentation prior to your appointment helps avoid delays and ensures the evaluation process moves smoothly.

Please note that incomplete documentation may delay completion of the evaluation or final recommendations.

DUI EVALUATIONS

A date to appear in front of a judge is scheduled.

  1. IT IS YOUR CHOICE  to be proactive and choose your own DBHR Licensed Evaluator* and treatment provider for your drug/alcohol
    assessment and treatment before your 1st court hearing; or wait for your 1st court hearing and the judicial system may tell you where they want you to go.

  2. If you choose RockWater, we would need you to bring any tickets issued, Notice of Summary Suspension and your Driver's Abstract from Secretary of State (DMV).

  3. Evaluations take approximately 60-90 minutes.

  4. You will receive a copy of your Evaluation within 1-2 business days with the appropriate recommended treatment level. You will receive (2) copies of your Evaluation at no additional charge. Present your Evaluation and treatment recommendation to your judge.

*In the state of Illinois, Cook and DuPage Counties' initial DUI Evaluations must be completed by their approved Evaluators. In some cases, you may be required to obtain your initial evaluation at a location approved by the court system. DBHR is the IL Dept of Health, Division of Behavioral Health and Recovery department.

RWCC provides other services as well. If you have completed partial treatment at another facility, you can transfer your hours to RWCC.

BAC - WHAT IS THAT?

The Intoximeters “Drink Wheel” is a form that you can fill out that when completed and it will instantly compute your estimated blood/breath alcohol concentration (“BAC”) or blood alcohol content.  This simulates the breath alcohol testing done by an Intoximeter Alco-Sensor FST.  The Alco-Sensor FST is a screening instrument that can be used in both Law Enforcement and Workplace breath alcohol testing.

Intoximeters Drink Wheel is presented as a public service to Intoximeters web site visitors. Its primary purpose is to provide useful information about the responsible use of alcohol.

DRUG & ALCOHOL ASSESSMENTS

If you have been arrested due to an underage possession of alcohol, public intoxication, received a drinking ticket or possession of an illegal drug you will be court ordered to complete a drug/alcohol evaluation.

IT IS YOUR CHOICE to be proactive and choose your own SUPR Licensed Evaluator* and treatment provider, or you can wait until the judge orders you where to go.

We conduct several screenings and a full comprehensive assessment for treatment. After your interview, a recommended amount of treatment hours is identified. The treatment could fall in the ASAM 0.5, ASAM 1 or ASAM 2.1 and higher for the level of care needed to address an alcohol or drug substance use disorder.

How to obtain required documents for DUI Evaluations

ABSTRACT (Driving Record)
If you have a physical copy of your Illinois Driver's License, you can order your Court Purposes Abstract from ilsos.gov.
If you do not have a physical copy of your Illinois Driver's License, you will need to visit a local DMV branch and order in person.

NOTICE OF SUMMARY SUSPENSION/REVOCATION OR POLICE REPORT/SWORN REPORT
